*Please note on the sponsor registration form you are allowed one golfer for the $500 sponsor hole so please add the name and information of the golfer and shirt size, the golfer will also get a dinner. As a $500 sponsor hole, you will also get one extra dinner/reception ticket. Please email the name of the person who will be attending the dinner at dom@mcar.com
